I hereby permit the release of my son/daughter's  high school records from any high school they attend, including but not limited to grades, individual education plans, and standardized test scores to The University of New Mexico-Valencia Campus Upward Bound Program for use by the program and for the purpose of compiling and reporting data to the U.S. Department of Education, Office of Postsecondary Education. I also give my permission to the Upward Bound Staff to visit with instructors counselors, and administrators to obtain information concerning academic progress and disciplinary action, and to receive copies of report cards and transcripts.
